Information we collect

Information you provide to us. When you contact us, request an appointment, or otherwise communicate with us through the Site, we may collect information such as your name, email address, phone number, state of residence, the reason for your inquiry, whether you are a veteran, and any other details you choose to include in a message or form.

Information collected automatically. When you visit the Site, we and our service providers may automatically collect certain information about your device and usage, such as your IP address, browser type, operating system, referring pages, pages viewed, links clicked, and the dates and times of your visits. This information is collected through cookies and similar technologies.

Appointment scheduling. Our online scheduling and patient intake are handled through a third party health technology provider. Information you submit through those tools is collected and processed by that provider under its own terms and privacy and security obligations, in addition to ours.

We provide clinical services to patients of various ages, including minors, but appointment requests and other forms submitted through the Site should be completed by a parent, legal guardian, or another adult acting on the patient's behalf. We do not knowingly collect personal information directly from children under 13 through the Site. If you believe a child has provided us information through the Site, please contact us and we will take appropriate steps to remove it.

The Site may contain links to third party websites and tools, including our scheduling provider. We are not responsible for the privacy practices or content of those third parties. We encourage you to review the privacy policies of any third party site you visit.

Do Not Track

Some browsers offer a "Do Not Track" signal. Because there is no common industry standard for responding to these signals, the Site does not currently respond to them. We will update this policy if that changes.
